Family Medicine
Our patients depend on us to provide this high quality and affordable services. We offer a wide variety of services that are fully personalized and administered by our Nurse Practitioner who specializes in Family Medicine.

About Me
Desireé Samples, originally from Northwest Illinois and now residing in Northwest Indiana since late 2015, is a dedicated Board-Certified Family Nurse Practitioner. With more than 12 years of experience in this role, she brings a wealth of expertise to her practice. As a devoted wife and mother of three children—Eric (14), Addelin (11), and Colten (8)—Desireé actively engages in their academic pursuits and sports activities. Her family bonds over shared interests, including baking, hosting gatherings, hiking, playing games, and exploring new destinations. Additionally, Desireé has a compassionate side, evident in her passion for rescuing animals, as she lovingly cares for her puppy, Winnie, and her cat, Bruno.
Over the course of her 18-year career, Desireé has provided comprehensive care across all age groups, including pediatrics and adult critical care. Her experience spans diverse healthcare settings, such as retail health, psychiatric telehealth, hospice and palliative care, and private practice. Currently, she is pursuing full-time doctoral studies, on track to graduate with a Doctor of Nursing Practice (DNP) degree in December 2024.
Desireé's vision for the Connection Medical Clinic took shape over half a decade ago when she identified a pressing community need for healthcare that marries Christ-centered values with a commitment to excellence and affordability. This need encompasses individuals with or without insurance coverage.
At the heart of Connection Medical Clinic's mission is the aspiration to unite the body, soul, and spirit. By delivering Christ-centered services, we aim to enhance patients' health and overall well-being, all while connecting them with tailored resources that guide them on their journey to wellness.

The Connection Center
A Place Of Christ Centered Community Outreach
Phase 1 of The Connection Center includes our newly remodeled facility located next to Calvary Assembly of God Church. Our facility focuses on community outreach with a mindset of ministry to our surrounding community. Our current programs include homework help, counseling services, social services, banquette hall/catering, senior fitness programs, Zumba class, & a monthly activity in support of the Phil's Friends cancer outreach ministry (assembly of care packages, donations, decorating shipment boxes, and creating cards that get sent out to cancer patients monthly).
Phase 2 Stage of The Connection Center is also underway! Phase 2 will include even more opportunities for our community. We are looking forward to being able to play basketball, soccer, & volleyball at our new fitness center. There will be fitness rooms for physical training and classes in conjunction with a rehab facility. There will be a full-service café with a commons area for relaxation before or after a workout. The upstairs will have a padded walking/running track for those who enjoy low impact exercise. More information to come soon on our expansion!

We feel that we are called to be the hands and feet of Jesus by meeting people where they are and walking through the fire with them with love. The Social Services program at The Connection Center serves the purpose to meet people’s need for resources and community engagement. When you come into the Social Services office you will be met with a competent and professional staff member. They will walk alongside you wherever you are in life by listening to your needs or concerns and research solutions that will best meet your needs. Our heart is to also grow as we go! We want to grow where the need is in order to best serve our community and the wonderful people living in it.
Services currently offered:
Case Management: Someone to walk alongside of you as you navigate a need for services. We will help you develop positive skills and empower you in your own life, while assisting you in accessing resources.
Resource referrals at the local, state, and federal level.
Assistance in accessing government programs and resources.
Community Engagement: Building bridges in the community and providing educational opportunities for social issues.
We are consistently seeking new ways to serve youth, family, and individuals within our community. Be on the lookout for expanding services in the near future!

Homework Help

Our Homework Help service is offered to students during the academic year to those who may need a little extra help to achieve the academic success they are wanting to reach. Tutoring will be available on Tuesdays from 5-7 PM.
Students will have to sign-in and sign-out of the sessions. There will also be a rules & expectation form that is required to be signed by a parent/guardian. This is group learning setting, this is not a one-on-one tutoring session. If you are wanting additional one-on-one help, it can be arranged (fees apply).
Our staff will include teachers and peer adult/student assistants with the mission of helping your student become confident and successful in their course work.
