Dear Valued Patients and Community Partners,
With prayerful hearts and deep gratitude, I am writing to share an important update regarding Connection Medical Clinic.
After much prayer, reflection, and discernment, I have made the difficult but faith-led decision to close the in-person charitable services of Connection Medical Clinic, effective February 28th, 2026. It has been a tremendous blessing and honor to serve this community, and I am forever grateful for the trust you have placed in this ministry and in me as your provider.
Connection Medical Clinic was founded on a calling to provide compassionate, Christ-centered care to those in need. Every patient encounter, every prayer shared, and every life touched has been a testimony to God’s faithfulness. While this chapter is coming to a close, I firmly believe that God is continuing to lead and expand this calling in a new direction.
I am transitioning my clinical focus to WeightWise Virtual Clinic, LLC, a fully virtual healthcare practice devoted to evidence-based weight management and metabolic health. Through this platform, I will continue serving others with the same heart, integrity, and commitment to honoring God in healthcare, while reaching patients in a broader and more sustainable way.
Please note that WeightWise Virtual Clinic, LLC is a separate entity and operates independently from Connection Medical Clinic. All services are provided via telehealth and are subject to clinical appropriateness, licensure, and state regulations.
For patients who require ongoing primary care or services outside of weight management, I encourage you to establish care with a local primary care provider or community health clinic.
Thank you for allowing me to walk alongside you in your healthcare journey. It has been a sacred privilege to serve this community. Though the structure of this ministry is changing, my heart for service remains the same, and I trust God’s hand in each step forward.

About Me
Dr. Desireé Samples, DNP, RN, FNP-BC originally hails from Northwest Illinois and has resided in Northwest Indiana since late 2015. She is a dedicated Board-Certified Family Nurse Practitioner with more than 15 years of experience as a nurse practitioner. She prides herself on bringing a wealth of knowledge and compassionate care to her practice.
She is a devoted wife and mother of three children to Eric (16), Addelin (14), and Colten (10). Her family bonds over shared interests including hiking, games, and exploring new destinations. Additionally, Desireé has a passion for rescuing animals, she lovingly cares for her Cockapoo Winnie, and rescue cat Bruno.

Over the course of two decades as a nurse, Desireé has provided comprehensive care across all age groups. Her experience spans diverse healthcare settings, including pediatric intensive care, transplant intensive care, retail health, psychiatric services, hospice and palliative care, cardiovascular and vascular surgery, and private practice.​
